Improve autonomous implementation review convergence

Summary

• replace implementation-phase code verification with convergent code review covering approved outcomes, unsupported scope, and material code quality
• constrain security and integration-test findings to required corrections and route accepted findings through minimal, bounded fixes
• add an evidence-backed quality profile recipe that proposes repository-specific criteria and waits for explicit confirmation before writing
• align review reruns and resolution handling across build, implementation, and standalone review recipes
