Write each saved checkpoint straight to .vtu and refresh the .pvd collection, instead of converting the whole .sol series at the end of the run.

This PR adds a
save_vtu_each_timestepvisualization option for transient simulations.When enabled, each saved checkpoint is immediately written to .vtu instead of converting the complete .sol series at the end of the simulation. The corresponding .pvd collection is updated after each output step. Existing .pvd entries are preserved when a simulation is resumed.
