A request whose handler throws is judged by the response of the error handler
that answers it. When no error handler is registered, the middleware sees no
response at all and counts the request as not successful, whatever its status.

### `store`

The `Store` to use to store the hit count for each client.

Calculate skipped-request quota from the post-decrement count.

When another request increments the same key before this request finishes, totalHits is stale. The decrement updates the shared count, but the handler uses that snapshot to set current and remaining. This can report an extra slot compared with the store.

Make Store.decrement return the post-decrement count atomically, then use it for both values.

Review comment at @packages/rate-limit/src/handler.ts around lines 107 - 109:
Update the Store.decrement contract and its implementations to atomically return
the post-decrement hit count. In the handler’s skipped-request path, use that
returned count for both the current and remaining rate-limit values instead of
the stale totalHits snapshot.
