ElevenLabs can reject speaker transcription with HTTP 401 for quota or access problems. These failures previously became retryable 502s, obscuring the cause and repeatedly calling the provider.
Classify provider failures into terminal billing/access errors, apply bounded cooldowns across LiveVoice operations, preserve refunds on rejected requests, and log safe usage/error metadata without audio, transcript text, or credentials. Document the existing persistent spending controls.
Validation: 26 focused backend/client tests passed; one existing PostgreSQL integration test skipped because its dedicated test database was not configured. Full workspace typecheck and backend build passed. Production-key transcription diagnostics succeeded locally and inside Railway.
