Update Neki extensions

Set enabled extensions and their parameters in one configuration profile update.

pscale branch config-profile update app main default --org acme --format json \
  --extensions=hll --parameters pgconf.hll.force_groupagg=on

Omit --extensions to leave the enabled set unchanged. Use --extensions= to disable all customer-managed extensions.
