LCORE-3788: keep a turn a shield blocked on /v1/responses out of what compaction sends to the model - #2996
LCORE-3788, part 1 of 2. Based on main; part 2 builds on this PR.
Bug. When a shield blocks a /v1/responses request, the turn is stored: the input and the refusal. Compaction rebuilds the model's context from the stored items. It skipped only tool items and summary markers, so the next compacted request replayed the blocked text verbatim, and a later compaction folded it into a summary.
Fix, three commits:
A mark for the messages of a blocked turn: an item id that starts with msg_blocked_ (src/utils/blocked_turns.py).
Compaction leaves marked messages out of the replay, the summarizer input and the token estimate.
PendingTurn.store_blocked stores the turn with the mark. /v1/responses already reaches it in every mode, so the endpoint does not change.
Decisions:
The mark is the item id. Items have no metadata field, but OGX keeps an id the caller supplies. The ids are random (ids are unique across conversations) and replace any id the client sent.
The whole turn is filtered, the question and the refusal.
GET /v1/conversations/{conversation_id} still returns blocked turns. Only what the model reads is filtered.
No migration. Turns stored before this change carry no mark.
A marked message is never a summary marker, even when its text starts with [lightspeed:compaction-summary].
[covers:N] still counts blocked items. It indexes the stored items, so the filter runs after the cut.
Does not close the ticket./v1/query, /v1/streaming_query and A2A run shields inside the agent; those turns stay unmarked until part 2. Not a regression: they are unmarked on main today.
Not covered here (part 2 documents the full list):
A blocked first turn of a conversation that is not compacted: OGX builds that context itself and ignores the mark.
The topic summary of a new conversation is generated from a blocked first query.
A client can send its own input message under a msg_blocked_ id and hide it from compaction (read from the code, not tested).
The design doc and user guide change in part 2.

Testing
Reproduce on a live service. Start it in library mode with compaction on (context_windows: openai/gpt-4o-mini: 2000, threshold_ratio: 0.1, token_floor: 100, buffer_turns: 1) and the topic-guard question-validity shield from tests/e2e/configuration/library-mode/lightspeed-stack-shields.yaml. Point the OpenAI provider's base_url at a proxy that records every request OGX sends. On one /v1/responses conversation send two long turns, one compacted turn, a blocked turn holding a made-up code, then two follow-ups.
Expected: the blocked turn is stored under msg_blocked_ ids; the history returns it; no request to the model holds the code except the shield's own classification.
Result: as expected (token ZEBRA-4471 is in no request other than the shield's own classification of the blocked turn). On the base the code reached the follow-up chat request, the summarizer and the request carrying that summary.
A turn blocked before compaction: stored marked, kept out of the first summarizer request, counted by its marker ([covers:4] over 2 summarized items).
With a 128000 window (no compaction), a blocked first turn still reaches the model through OGX and the topic summary, as "Not covered" says.
The live run used the whole series (parts 1 and 2), before the rebase onto the current main. For /v1/responses that is the code of this PR.
Run the tests of this change:
uv run pytest tests/unit/utils/test_blocked_turns.py tests/unit/utils/test_conversation_compaction.py \
tests/unit/utils/test_conversations.py tests/unit/utils/test_pending_turn.py \
tests/unit/app/endpoints/test_responses.py -q
uv run pytest tests/integration/endpoints/test_compaction_conversations_api.py \
tests/integration/endpoints/test_turn_persistence.py -q
Ten unit tests and two integration tests are new. The regression test the ticket asks for is test_blocked_turn_is_not_sent_to_the_model_by_the_next_request (blocking and streaming): block, then follow up on a compacted conversation, and the second request's input holds neither the blocked text nor the refusal.
Run the full suites:
uv run pytest tests/unit -q # 3941 passed, 1 skipped
uv run pytest tests/integration --ignore=tests/integration/container_lifecycle -q # 373 passed
Run the linters: ruff, black, pylint (10.00) and pyright (0 errors) pass on the touched files. The e2e suite was not run; no e2e scenario is added.
Summary by CodeRabbit
New Features
Blocked messages are now clearly marked when stored, while remaining visible in conversation history.
Bug Fixes
Blocked messages are excluded from future model input and summarization, so they do not affect conversation compaction.
Messages that begin with the summary-marker text are no longer mistaken for compaction markers when they are marked as blocked.

Blocked turns receive marked message IDs when stored. Conversation history retains those messages, while compaction excludes them from model input and token estimates.
The append path accepts a blocked flag. PendingTurn.store_blocked passes blocked=True; tests verify marked storage, follow-up request input, and returned conversation history.
Compaction rejects blocked items as markers and filters them after the marker boundary. Token estimates and summarization exclude blocked items, while marker coverage still counts stored items.

Explanation
Docstring coverage is 100.00% which is sufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 16 functions across 11 files.
Full details: Performance And Algorithmic Complexity
Explanation
No meaningful performance regression is introduced. The new blocked-item helpers perform single linear scans with O(1) ID checks, and mark_blocked performs one linear pass over a turn. Compaction adds only these linear filtering passes before existing token estimation. No nested scan, per-item API/DB call, unbounded buffer, or new list operation is introduced. get_all_conversation_items already paginates through all pages, and that implementation is unchanged.
Full details: Security And Secret Handling
Explanation
No security or secret-handling violation is introduced. The new ID uses secrets.token_hex(24) and no credentials or tokens are logged or hardcoded. The PR changes compaction and blocked-turn persistence only; it adds no API endpoint or authorization path, SQL/command/path input, or response/logging path. No Kubernetes Secret manifests are changed. Blocked turns remain visible in conversation history by design, but that existing API behavior is not introduced by this diff.

A turn a shield blocked has to stay in the conversation, so the history
is complete, and has to stay out of what compaction replays to the
model. Nothing tells the two apart today: a blocked message is stored
like any other.
A conversation item has no metadata, so the mark is the item id. OGX
keeps an id the caller supplies, and the ids it generates are msg_
followed by hex digits or a UUID. utils/blocked_turns.py holds the
prefix (msg_blocked_), a generator for new ids, the predicate, a filter
for the code that builds the model's context, and mark_blocked, which
gives every message of a turn a new blocked id before it is stored.
Nothing uses the module yet, so behaviour does not change.
Tests: tests/unit/utils/test_blocked_turns.py, one test per function.
The predicate is also checked on the type OGX lists items as.
Compaction rebuilds the model's context from the stored conversation
items. It skipped tool items and summary markers, and nothing else, so
a turn a shield had blocked was replayed verbatim on the next compacted
request, and folded into a summary once it left the buffer.
_load_compaction_state now drops the items that carry the blocked mark
from the recent items, which feed the token estimate, the partition,
the summarizer and the explicit input. needs_compaction_path leaves
them out of its estimate. The filter runs after the stored items were
cut at the count the last marker records: that count stays an index
into the stored items, so _covered_item_count gets the unfiltered list.
A marked message is never read as a summary marker (is_marker_item). A
blocked text with the marker prefix would otherwise be sent to the
model as a summary and move the boundary of the recent turns. The
conversation read therefore returns such a message, where it left out
every user message with the prefix before; the docstring of
exclude_marker_items names the exception. Nothing stores a turn with
the mark yet.
Tests, unit: blocked pairs before and after the boundary are absent
from the explicit input (also fails with the filter ahead of the cut);
a blocked text with the marker prefix is no summary, no boundary, and
exclude_marker_items keeps it; the summarizer does not get a blocked
turn and the marker says [covers:4], the raw index; the estimate of
needs_compaction_path does not count it. All four fail without the
change. Integration: the conversation read returns a blocked turn,
and fails when the handler leaves marked items out.
/v1/responses stores the turn of a request a shield blocked: the input
as it arrived and the refusal. On a compacted conversation the next
request replayed both to the model, so the shield only delayed the
text by one turn.
append_turn_items_to_conversation takes blocked=False. When set, every
message of the turn is stored under a blocked id. PendingTurn sets it
in store_blocked and nowhere else, so a completed or an interrupted
turn is stored as before. /v1/responses reaches store_blocked in every
mode (compacted or not, with a previous response), so the endpoint
itself does not change. Together with the filter of the previous
commit, a blocked turn stays in the conversation and is neither
replayed nor summarized.
Turns that were stored before this commit carry no mark and are still
replayed.
Tests:
- integration, test_turn_persistence.py: a request blocked by a shield
and then a follow-up on the same compacted conversation, blocking
and streaming. The store holds the blocked input and the refusal
with ids starting msg_blocked_, the follow-up turn is not marked,
and no item of the second request's input holds the blocked text or
the refusal. With the filter taken out, the test fails on the
request body.
- unit: the write marks every message of a blocked turn; of the three
ways the owner stores a turn, only the blocked one is marked.
- three assertions in the /v1/responses unit tests follow the new
argument.

Use explicit input when blocked items exist without a summary marker.
When a conversation has blocked items, no summary marker, and no cached summaries, _load_compaction_state removes them from recent_items. If the follow-up stays below the threshold, apply_compaction returns unchanged parameters with conversation intact. /v1/responses passes those parameters to OGX, so the blocked items can be reloaded for the model.
The regression test does not cover this case. It enables compaction but seeds _compacted_conversation(), which already contains a summary marker. Add a below-threshold follow-up test that uses _plain_conversation() with compaction enabled. Ensure blocked items trigger explicit input even when no summary is needed.
