修复 Terminal 重挂载历史重放的 xterm 自动响应泄漏：terminalStream 现在原子返回历史快照并订阅实时消息；新增 replay/live 协调器，使用 xterm write completion callback 完整处理历史后才注册 onData，并在注册后按序刷新重放期间缓冲的实时输出。卸载会取消订阅并阻止延迟注册/PTY 写入，TerminalInstance 仍沿用 registry 的 xterm 释放流程。每个挂载会话使用独立流式 TextDecoder，避免跨重挂载的分段 UTF-8 状态污染。保持 1 MiB 历史预算、生命周期状态和 close 清理语义不变。新增行为级回归覆盖历史响应抑制、重放后键盘输入、实时查询响应、重放期间实时输出顺序、UTF-8 分块与重挂载、重放中卸载及生命周期历史。
