All scanned-site text HTML-escaped; long values truncated; observation timeouts reported; generated even when scan fails. - action structure tests: 21 (v0.5.1) + 21 (v0.6.0) = 33 total - gate semantics integration tests: 6 total - 160/160 tests green, npm audit 0
