Summary

#5979 added cli/t27b to the workspace members but not to docs/reports/orphan_modules.json. Master now fails two checks:

Every source file is reachable from its crate root: cli/t27b: no ceiling in docs/reports/orphan_modules.json.
cli-tri: modreach::tests::the_ratchet_watches_every_member_and_no_ghost, which requires every workspace member to have a ceiling.

The gate's own measurement for the crate is 11 files under src/, all 11 compiled, 0 orphaned (+2 reached via #[path]). The ceiling is therefore exactly 0, as the ledger's exact-match rule requires.

Closes #5988.
