agent-provider-chain: reserve configured live, a skipped provider is logged

• RESERVE_CONFIGURED_LIVE=true: 2026-09-15, GET /api/agent/provider on vibee-render e2c4617 lists reserve(deepseek-chat) after nemotron.
• RESERVE_ANSWERED_LIVE stays false: with reserve first, one tools_only probe was answered by zai; RESERVE_PROBES=1.
• New rule SKIPPED_PROVIDER_IS_LOGGED / SKIP_LOG_PREFIX: streamModel warns with the diagnose() reason on every fall-through. Host PR in 999-multibots-telegraf follows.
• No new test blocks (pin 12758 unchanged); docs/now entry added.
