fix: keep the reaction cap when reactions arrive at the same moment - #1142
A person can leave at most 12 reactions on a Remote message (MessageReaction::MAX_PER_MESSAGE). MessageReactionLimit checks this while validating the request, by counting the reactions already saved. Then CreateMessageReaction inserts the new one.
When several reactions are sent at the same moment, every request counts before any of them has saved. Each one sees fewer than 12, so each one passes and inserts.
I reproduced it locally with 30 PHP processes started together. Each one validated with MessageReactionLimit and then called CreateMessageReaction with a different emoji on the same message. Over 5 runs, 30 reactions were saved every time, against a limit of 12. This takes nothing more than a script that sends the requests in parallel.
The fix
CreateMessageReaction already runs in a transaction. It now:
locks the message row (lockForUpdate), so reactions to the same message are saved one at a time;
counts the person's reactions again inside the lock;
stops without saving when they already have 12 and the new emoji isn't one of them.
The validation rule stays, so a normal request over the limit still gets the usual 422 error. The check in the action only catches the requests that passed validation at the same moment. Re-sending an emoji you already used is still a no-op, as before.
Tests
New test keeps the cap when reactions sent at the same moment all passed the check: it calls the action directly, as a request that had already passed validation would. A 13th emoji is not saved, re-sending one of the 12 still works, and another person's reaction on the same message isn't affected. It fails on production and passes with the fix.
Same 30-process script with the fix: 12 reactions saved in each of the 5 runs.

Bug Fixes
Message reactions are limited to 12 distinct emojis per person on a message. Additional new reactions beyond that limit are not added, while existing reactions and reactions from other people remain unaffected. The limit is checked during the update, so simultaneous reaction attempts also respect the cap.
The message reaction action now locks the conversation and checks a user's existing reactions within a transaction. It blocks new emojis when that user reaches the reaction limit. A test checks the limit and confirms that another user can still add a reaction.
The action locks the conversation and checks the user's existing emojis before inserting. The test verifies that a user with 12 reactions cannot add new emojis, while another user can add a reaction.
dev-null-sh/dev-null.sh#1187: Establishes conversation-first lock ordering for message mutations, which this action follows.
dev-null-sh/dev-null.sh#1101: Uses the conversation lock and updates the message reaction version, which this action also updates.

Test competing transactions for the reaction cap.
The current calls run sequentially. Each CreateMessageReaction::handle call completes its transaction before the next call starts, so the test still passes if lockForUpdate() is removed. Use separate database connections or concurrent workers that add reactions to the same message, then assert that no more than 12 reactions remain.

Locking the message first reversed the order pins and removed reactions use (conversation, then message), so a reaction added while another was removed or the message pinned could deadlock.
