Block j/k while a page switch is in flight - #32

After j advanced to the next page, the old list stays displayed (stale) until the fetch lands, so another j at the last index stacked a second page advance and jumped to page 3. next_story/prev_story now ignore input while the displayed list is stale or a fetch is loading, and the page advance is also blocked during a same-page refresh (mid-list selection still moves then).
