Remove three unused private direct-completion helpers. Each already raised direct_ai_completion_forbidden and had no production caller; the active gateway path is unchanged. Failure tests install an explicit canary without relying on the deleted placeholders and still prove no fallback is invoked. No permission, trading configuration, dependency, deployment, server or actual secret/variable changes.

Validation: 66 AI audit tests passed with the existing pinned SDK installed; diff whitespace checked. Pure dead-code removal.
