What changed

The malicious code scan is no longer a workflow of its own. It is now a job inside AI Review that runs after the gate and before the review: gate → scan → review → publish (ai-review-run.yml).

  • New ai-review/ai_review_scan.sh fetches the PR's commits into a checkout of the base branch, reads them as data, runs malicious_code_scan.py, and decides whether the review may go ahead. The review job runs only when the scan reports passed=true.
  • When the scan blocks, the job fails and the PR's AI review comment says the scan blocked it, with a link to the findings in the run summary. It also carries a <!-- ai-review-scan-blocked: <sha> --> marker. The gate skips that commit on later CI completions, so the same Claude scan isn't paid for twice.
  • To override, a maintainer with write access adds malicious-scan-override and runs AI Review by hand. The label counts only if it was added after the "blocked" comment for that exact commit. Otherwise it is removed and the comment says why.
  • New inputs scan_project_description and scan_generated_paths on ai-review-reusable.yml replace the scan template's inputs. claude_model now applies to both the review and the scan.
  • Removed:
    • malicious-code-scan-reusable.yml, plus the malicious-code-scan template and its properties file
    • scan_record.py
    • the gate's wait for the scan, and the security/malicious-code-scan commit status
    • the scan_workflow_name, scan_wait_minutes and scan_status_context inputs
    • the statuses: read permission

Why it changed

The scan ran with every push, PR edit and label, and each run paid for a Claude review, including on PRs that would never be AI-reviewed. Now it runs only for a PR the AI review is about to hand to the agents, which is the case the scan is there to protect.

Testing strategy

  • python3 -m unittest discover -s tests: 64 tests pass. There were 89; 34 tests for the standalone workflow, its status records and the gate's scan wait were removed, and 9 were added.
  • The new tests run ai_review_scan.sh against a real git repo with a fetched refs/pull/1/head. They cover:
    • a clean pass
    • a block, with the comment created or updated
    • a block on the PR text alone
    • a moved head
    • an accepted override, and overrides rejected for each reason
    • the gate skipping blocked or already-reviewed commits
    • the job order in the workflow
  • ruff, shellcheck and actionlint are clean.

Review notes

  • Breaking for callers:
    • Repos with the old malicious-code-scan.yml must delete it, because the reusable workflow it calls is gone.
    • They must also drop security/malicious-code-scan from required checks.
    • Callers passing the removed inputs will fail. The template had scan_wait_minutes commented out.
  • Merge this before the cosmos PR, which passes the new scan_* inputs.
  • The override check is weaker than before. It now relies on a github-actions[bot] comment, the same check used for "already reviewed", instead of the old verified status record. A workflow on a branch of the same repo could post a fake "blocked" comment. A maintainer still has to add the label after it, and anyone who can push such a workflow can already read the repo's secrets.
  • Fork PRs are no longer scanned. The gate already skips them, so they were never reviewed.
  • Forced re-reviews lose their override. Once an overridden commit is reviewed, the review comment replaces the "blocked" comment, so a later forced re-review needs the label added again.

Summary by CodeRabbit

  • New Features

    • Pull requests are scanned for malicious changes after CI completes and before AI review begins. Blocking findings pause review until a maintainer applies the override label and starts a manual review. New pushes are scanned again.
    • Optional project-description and generated-path settings are available to configure scans.
  • Changes

    • The separate malicious-code scan workflow is no longer available; scanning is managed through the AI review workflow.

📝 Walkthrough

The AI review workflow now runs a malicious-code scan before review. Scan findings can block review until an eligible maintainer adds the override label and starts a manual run. The separate scan workflow and its status-record verification have been removed.

Changes: AI review scan flow

Layer / File(s) Summary
Integrate scanning into the AI review workflow
.github/workflows/ai-review-reusable.yml, .github/workflows/ai-review-run.yml, .github/workflows/malicious-code-scan-reusable.yml, malicious-code-scan/scan_record.py, workflow-templates/ai-review.yml, workflow-templates/ai-review.properties.json, workflow-templates/malicious-code-scan.*
The AI review workflows accept project-description and generated-path settings. They run a scan before review and require a passing scan for review to run. The separate scan workflow, status-record verifier, and scan template are removed. The remaining templates describe scanning and the override process.
Check scan results and maintain override state
ai-review/ai_review_gate.sh, ai-review/ai_review_scan.sh, ai-review/ai_review_publish.sh, malicious-code-scan/malicious_code_scan.py
The gate checks review comments and override labels instead of scan statuses. The scan script checks PR head freshness, runs the scanner, validates overrides, and creates or updates a review comment. The publish comment and scanner documentation are updated.
Validate gate, scan, and override behavior
tests/test_ai_review.py
Tests cover scan outcomes, stale heads, gate behavior, override validation, and workflow dependencies. Tests for the removed standalone scan workflow and status-record flow are deleted.

Merge Risk: ⚪ Minimal · up to 47c3e

The inspected scan configuration and stale-commit safeguards do not reveal a merge-blocking issue.

Security architecture risk: 🔵 Low · up to c8b27

The scan remains tied to the reviewed commit, and overrides still require privileged approval. The main risks are coordinating existing workflow and branch-protection settings with the retired scan, and ensuring override behavior matches the documented manual process. Deployed configuration remains unverified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • observed — An accepted override reaches the ordinary review and publishing path without granting additional job permissions. Publishing is documented to use a repository-scoped token without workflow-write permission, with fixes offered through a separate PR by default or pushed to the reviewed branch when configured. Actual externally configured token scopes remain unverified.

Security Findings and Attack Paths

  • observed — The canonical security assessment contains no retained findings and one deferred override candidate. Its missing source-bound verification receipt remains a coverage limitation, not evidence that an exploit exists or that the candidate is safe.

Trust Boundaries and Controls

  • observed — An override requires a currently present label, an exact-head blocked marker in a matching bot-authored summary, a later label event, and current write or admin permission for that event's actor. The comment selection checks bot identity and marker text, not workflow-specific provenance.
  • observed — A valid override can also be accepted on automatic CI completion: the gate lets labeled blocked heads proceed, and the scan has no manual-event requirement. This differs from the head's documented manual process, but the base already accepted privileged label events without a second manual approval, so no newly expanded approval authority is established.

Resilience and Maintainability Implications

  • inferred — Acceptance does not immediately consume the override label or replace the blocked marker, so interruption before publication can leave same-head authorization reusable. The base also persisted exact-head overrides across rescans; reuse alone therefore does not establish a new security regression. Runtime interruption and concurrency behavior were assessed statically, not demonstrated in execution.

Hardening Proposals

  • proposed — Coordinate retirement of standalone callers and required-status settings explicitly, preserving any intended merge-time malicious-code control independently of eligibility for automated review.
  • proposed — If manual execution is intended as an additional approval boundary, enforce it explicitly; otherwise document that a valid privileged label can authorize the next eligible automatic run.

Comment thread ai-review/ai_review_scan.sh
The override label is the maintainer's approval of the blocked commit. Any
later run for that commit accepts it, including one started by a CI
completion, not only the manual run the docs described.
