Typo in measurement - column should be unit_source_concept_id, not unit_source_id
The CDM datatype column uses a mixture of 'integer' and 'Integer'. All other datatypes only use lowercase.

Without the quote, CSV parsing results in an unwanted additional element for that row.
The skip guard checks CDM5_POSTGRESQL_SERVER, but the actual connection details use CDMDDLBASE_POSTGRESQL_SERVER. This causes tests to run (not skip) in environments where CDM5_POSTGRESQL_SERVER is set but CDMDDLBASE_POSTGRESQL_SERVER is not, leading to failures. Closes #719
