Centralizes the default release-tool PHAR version inside
actions/setup-release-tool/release-tool-version. Consumers can now pin only the immutable github-workflows action SHA; theversioninput remains available solely as an explicit override. Also removes the stale hard-coded release-tool version from the reusable release-plan action and updates tests.
