docs: sync the README and CLAUDE.md tool lists with the shipped tools - #108

file_create and folder_create shipped in #98 but were never added to the tool tables, leaving the README listing 72 of the 74 tools the server exposes. Both live in DocumentTools, so they go in the Document Tools table, which stays alphabetically ordered.
The MCP Tools Available section listed 38 of the 74 tools the server exposes. It was missing the Navigation, Debugger, Diagnostics and Window groups outright, along with build_configuration_get/set, document_save, document_cleanup, file_create and folder_create. The list is now generated from the README tables, so both documents describe the same tools with the same descriptions and are grouped identically. Sections follow the README's eleven groups rather than the previous four, which had folded the editor and selection tools into Document Tools. The terminal and coverage caveats are kept as plain prose, since CLAUDE.md does not use callout styling.

Brings both tool lists back in sync with what the server actually exposes. The server has 74 tools; the README documented 72 and
CLAUDE.mddocumented 38.I checked by extracting every
[McpServerTool(Name = "...")]fromServer/Tools/*.csand diffing against each document, rather than reading by eye. Both now match the code exactly, with nothing missing or extra in either direction.README — 72 → 74
