The brief maps every export to <module>::<name>, so a default export
became app::plugin::default, an item no Rust module can hold, while
done already skips default exports. The brief now says a default
export has no Rust item of its own and names the two --map forms
done accepts.

Testing: a new end-to-end test in tests/porting_workflow.rs fails on
main and passes with the change. cargo fmt --check, cargo clippy --all-targets -- -D warnings and cargo test pass.
