fix(gic): allocate a separate ITS ITT per DeviceID - #620

Maintain ITT state for each ITS and DeviceID pair so different devices cannot share EventID entries. Reuse an existing ITT for subsequent mappings, size it from the MAPD Size and GITS_TYPER.ITTEntrySize fields, and unmap the device only after its final EventID is discarded. Handle allocation failures, validate EventID bounds, and clean the ITT before publishing it to the ITS.
