fix(baremetal): allocate IOVIRT maps for all nodes - #603

Allocate flexible-array storage for the maximum number of mappings across every IORT node. This prevents the PAL from writing beyond the IOVIRT info-table allocation when multiple nodes contain mappings. Handle allocation failure before passing the table to VAL.
