v3 telemetry: per-file calibration features + coverage fields in harness output (E-class)

C1 needs per-file feature vectors in the harness output: raw score per mode, rank per mode, in-diff flag, top-source edge category, token size, discovery source. Emit them from the existing locate provenance into the per-instance records.

• E-class: no selection change; gated by scripts/bitcheck.sh (record before / check after).
• Also emits the C5 decomposition fields (gold ∉ universe / ∈ universe ∧ ∉ selection / selected).
• Step 3 of the order of work; blocks C1 and C5.
