Move external adapters under the adapters layer

Relocate provider, auth, cost, telemetry, perf, logging, upgrade,
changelog, crash, and inference helpers under src/adapters/. Colocate
list-dir.test.ts with its module under src/util/. Import path updates
only; no behavior change. Linear CL-6301.
