Subject: [PATCH] Set the Visual Studio extension's author to the Marketplace
 publisher name

The Visual Studio Marketplace only publishes an extension whose author
(the Publisher attribute of Identity in source.extension.vsixmanifest)
matches the publisher's display name, which is "AMD". It was "AMD -
Advanced Micro Devices", so publishing was rejected. The extension ID
is unchanged.
