Add debug logging to every action the bot takes

For example when adding a role, completing a command, or sending a reminder message.

A unique LoggerAdapter could be used at the top of every cog file, along with the format specifier including the command name, that way the command name does not need to be manually added to the context each time. This would also simplify the current process of retrieving the command name when sending error messages in TeXBotCog.command_send_error().

In addition it would be better to raise an error on any sort of failure to complete a command. Better custom exception classes would be created like CommandExitedNoChanges & CommandExitedInvalidIput. These would then be caught by the on_command_error event handler such that debug messages are sent always, error log messages are only sent if something goes badly wrong (not if the user just entered an invalid input), and a response is always sent.

New loggers would need to be created: TeX-Bot-commands, TeX-Bot-tasks & TeX-Bot-other that would be logged to and have the formatters take in the command/task name where necessary. During handling of the on_command_error event when logging, the command name will need to be manually passed to the logger after computing it from the current context.
